Online Enrollment Form

You will need a Google account to access the enrollment form linked below. When prompted, please sign into your Google account. If you don’t have one, please create a Google account.

Required Documents

After completing the online enrollment form and reservation fee of P10,000, we will send instructions for you to submit the following:

NSO Certified Birth Certificate (or equivalent if not a Filipino citizen)
Transcript of Records (from the last school attended)
Certificate of Financial Clearance (from the last school attended)
Certificate of Good Moral (from the last school attended)
